Immunizations Medications Other Information
All students are
required to have the following Immunizations
according to state policy.
DtaP -
5 doses required unless 4th dose was
received on or after
4th birthday. Students 7 yrs and older require 3 doses. One dose of
Td required ten years after the last dose of DtaP.
IPV - 4 doses required unless 3rd doese was received on or after 4th birthday.
MMR -
2 doses required with 1st dose received
on or after 1st
birthday. Second dose by age 5 or entry to kindergarten.
HEP B
- 3 doses required for the
following school years:
2004-2005 K-5 and 7-10
2005-2006 K-11
2006-2007 K-12
VAR (Chickenpox) - 1 dose on or after 1st birthday for the following school years:
2004-2005 K-4 and 7-10
2005-2006 K-5 and 7-11
2006-2007 K-12
2 doses required if vaccine received at 13 years or older
Hepatitis A - Two doses on or after 2nd birthday 5 for grades K-3 only.
Please remember to supply the school with a copy of any updated immunizations for your child's permanent record.
Medication that must be given during school hours should be brought to the school nurse. All medication, over-the-counter, and prescription must be in the original container. A permission form must be signed by a parent/guardian before medication will be given.
Vision and Hearing screenings are done on all Pre K, K, 1st, 3rd, 5th, and 7th grade students.
Scoliosis screenings are done on all 6th and 9th grade students.
Children must be sent home if they have a temperature of 100 degrees or are vomiting. Any child sent home must remain home until they are free from fever for at least 24 hours (without Tylenol).
